Spring 2013 PDA Day Verification Form (PDF Form file , 264KB)
For verification of attendance at the institutional Professional Development Activity (PDA) Day.
You will receive a confirmation email when your online form is received by Staff Development.
PDA Day Workshop Proposal Form (PDF file, 170KB)
For proposing a workshop/seminar for Professional Development Activity (PDA) Day. For more information, see the Call for Proposals. Proposals may be submitted online or by sending a hard copy to Staff Development.
If submitting an online proposal, you will receive a confirmation email when your online form is received by Staff Development.
Proposals for the Fall 2013 PDA Day are due by Friday, March 29, 2013.
Flex Program Forms
Flex Activities Verification Form (PDF Form file, 298KB)
For verification of completion of an approved flex activity.
You will receive a confirmation email when your online form is received by Staff Development.
Individual or Group Flex Activity Proposal Form (PDF file, 35KB)
For proposing an Individual or Group Flex activity. This form is for printing and filling out manually.
Menu of Activities Proposal Form (PDF file, 23KB)
For proposing a Menu of Activities workshop
for any constituency.

Group Departmental Student Learning Outcomes Workshop Proposal Form

For proposing a Group Flex activity for working on departmental Student Learning Outcomes. This form is for printing and filling out manually.
Intent to Complete a Pre-Approved Activity Form (PDF file, 18KB)
For submitting the intention to complete an approved flex activity between May 6 and June 30. Regular Faculty only.
Misc. Forms
Enrollment Fee Reimbursement (PDF file, 144K)
To obtain reimbursement for SRJC course fees when the program is open. Must be submitted prior to registration.
Workshop Evaluation Form (PDF file, 219KB)
For evaluation of professional development workshops or activities occurring other than on PDA Day.
