International Students Health Evaluation Information
Before being admitted to Santa Rosa Junior College you must receive a health evaluation by a healthcare provider within the last year. This needs to be submitted in English, preferably on the Santa Rosa Junior College Health Evaluation Form. The office of Admissions and Records provides you with the Health Evaluation Form. If you are in Sonoma County and need a provider to obtain a physical exam, click on Sonoma County Health Centers - Physical Exams for information. Part of your health evaluation consists of your immunization history. You must provide proof of two doses of Measles, Mumps, Rubella (MMR), and the Hepatitis B vaccine series. In addition, the Tetanus/Diphtheria vaccine is highly recommended for your own protection. Proof of freedom from active Tuberculosis is required. A negative Mantoux Tuberculin Skin test (PPD) within the past 6 months will fulfill this requirement. If you have a history of receiving the BCG vaccine more than a year ago, you still need to have a skin test done, to comply with current CDC guidelines. If you have a history of a positive Tuberculin Skin test (PPD), documentation of a negative chest x-ray within the last year is required. If you are in Sonoma County, you may receive tuberculosis clearance services at the Sonoma County Tuberculosis Clinic at (707) 565-4762. You also may receive a Tuberculosis Skin Test (PPD) or assistance with tuberculosis clearance at Student Health Services if you are admitted to the International Student program. |
