Athletic Programs Health Requirements Before a student-athlete can practice or compete on a Santa Rosa Junior College team he/she must complete a physical assessment and obtain clearance from the Student Health Services department. This is for the following reasons:
Physical assessments are provided for all SRJC athletes at Student Health Services, as pre-arranged at a specific time for each team. Your coach will inform you of the date(s) for this and will distribute the necessary forms for you to complete ahead of time. Your coach will review these with you before the team’s scheduled clinic. Please note: If a student athlete is not in the local geographic area prior to the physical assessments and would like to access forms ahead of time, please see “Athletic Forms” below to download them. If a student-athlete does not attend the scheduled physical assessment for some reason, then the physical must be obtained from another source at the student-athlete's expense. Exceptions for make-up assessments in Student Health Services are made only after consultation between the Athletic Trainer and the Director of Student Health Services. Documentation of the following immunizations is required at the time of the scheduled physical assessment. This may be in the form of an official immunization record (usually yellow cards), or documentation obtained from a doctor’s office or clinic that administered the vaccines. Please note that it may take weeks for an office to find records and send them, so plan on this well in advance of your scheduled team’s clinic. Sometimes high schools keep records of immunizations after a student graduates for a year or two, so getting a copy of this from them also is an acceptable proof of immunizations. If you are requesting records from a healthcare agency, physician’s office or high school, you may request that they FAX this directly to Santa Rosa Junior College’s Student Health Services office. The FAX number is (707) 524-1858, and the records should have the student-athlete’s name clearly indicated on it, as well as what athletic team they are trying out for, if possible. Required Measles, Mumps, Rubella (MMR) Immunity. Two MMR vaccines given one month apart after the age of one, or a positive titer (blood test testing for immunity). Tetanus (Td) Immunity. One Tetanus booster vaccine given within the last ten years. Other immunizations are recommended to fully meet adult immunizations standards, but not required. This is a good opportunity, though, for an athlete to receive needed Hepatitis B shots or other recommended immunizations. The following is a list of the forms needed for athletes to complete prior to their physical assessments. These will be distributed to each student-athlete, by the coaches, prior to the team’s scheduled health. If necessary, you may also download these forms for printing, as indicated. If a student-athlete is a minor (under age 18) a Minor Consent form needs to be completed and signed by the parent/guardian prior to their physical assessment.
