Please go to the Student Health Services Website http://www.santarosa.edu/for_students/student-services/student-health-services/download-forms/ to download any health screening/physical forms you may need.

If you miss your date, you will have to pay for an outside physical (filled out on the SRJC Student Health Services form only). Contact your coach ASAP if you have a scheduling conflict. Health Screenings will be located at Student Health Services in the Race Building on the Santa Rosa campus.
